We will process your form in 1-2 business days. 


Once completed, we will email you with an invite to the patient portal. Please register in the Charm portal and check your messages. We will have the cost of your appointments available for you to see and the next dates and times for new patient appointments. Please call 541-516-6357 to speak with John, my office manager, if you have questions about accessing the Charm portal, your insurance benefits, available intake times, or any other questions.
